P-132 MANAGEMENT OF VENTRAL HERNIA IN OBESITY: OUR ALGORITHM

Abstract Introduction Management of ventral hernia in obese is a complex problem. The methods weight loss, alternatives if the patient cannot undergo bariatric surgery, timing, and type surgery lacks clarity are dependent on resources expertise. There need for algorithms based local population In this paper, we present outcomes our institutional algorithm. Methods It was retrospective analysis prospectively collected data. Patients with body mass index(BMI) > 30Kg/m2 were included to as per algorithm considering (a) presentation(symptomatic vs asymptomatic), (b) characteristics(defect width, site, reducibility), (c) obesity characteristics(BMI, subcutaneous fat, android gynecoid). Data age, BMI, comorbidities, tobacco consumption, location, contents, previous intraoperative parameters(the mesh, drain, fixation), outcomes(seroma, hematoma, infection, recurrence) collected. Results 50 patients underwent treatment Mean BMI 36.6 ± 7.3 kg/m2. mean follow-up 17.6 7.2 months. defect width 4.8 2.9 cm. two (4%) recurrences who an anatomical repair under emergency conditions. None elective had recurrence. Total surgical site occurrence 12% requiring procedural intervention 8%. one (2%) mortality postoperative day7 due myocardial infarction. Conclusion has shown encouraging results short-to-medium term. Long-term evaluation higher number needed confirm its usefulness.
